1. SERVICES OFFERED
  • Childcare services for children ages [X] months to [X] years
  • Hours: [Monday to Friday, 7:00 AM - 6:00 PM]

2. ENROLLMENT & PAYMENTS
  • Completed enrollment form required
  • Monthly fee: $[X] - payments in advance
  • Security deposit: $[X] (refundable with 30 days notice)
  • Late payments: $[X] fee after the [X]th of the month
3. CANCELLATION POLICY
  • Required notice: 30 days in writing
  • No refunds for unattended days
  • In case of emergency closure, credits will apply
4. HEALTH & SAFETY REQUIREMENTS
  • Children must be up to date with vaccinations (Florida DH 680 Form)
  • Children with fever or contagious symptoms will not be accepted
  • Medical authorization required for medications
5. PARENT RESPONSIBILITIES
  • Provide updated emergency information
  • Designate authorized pickup persons
  • Notify schedule changes 24 hours in advance
6. LIMITATION OF LIABILITY
  • Not responsible for lost personal items
  • In case of medical emergency, we will contact emergency services and designated contacts
